Through this new partnership, KHA and ChartSpan will work together to identify providers that can benefit from CCM, a turnkey, value-based care and reimbursement program, and Annual Wellness Visit (AWV) services. ChartSpan can provide their services to Kentucky's critical access hospitals and clinics as well as standard fee-for-service (FFS) and specialty practices.
